We're seeking a dedicated Clinical Case Manager to join our growing Value-Based Care Services team. In this remote role, you'll play a vital part in enhancing the quality of member management, maximizing satisfaction, and promoting cost-effective healthcare solutions. You'll work closely with clients and members to empower them to achieve their personal health-related goals.

What You'll Be Doing:

Data Analysis & Member Eligibility: Dive into data to identify eligible members for our Population Health Management Program.
Care Coordination: Ensure timely, effective, and member-centric care, following established healthcare processes.
Case Management: Manage case assignments from outreach to closure, ensuring thorough documentation and monitoring progress.
Collaboration & Reporting: Meet reporting standards and participate in collaborative meetings with team members and clients.
Member Empowerment: Assist members in reaching wellness and health autonomy by addressing barriers, social determinants, and psychosocial issues.
Healthcare Navigation: Educate members on navigating the healthcare system, promoting quality and cost-effective interventions.
Operational Support: Support operational aspects of the division to meet customer requirements and satisfaction.
Confidentiality: Maintain strict confidentiality of all computer programs, medical records, and data.
Quality & Utilization Management: Participate in QM/UM committee meetings (if applicable), including material preparation, data analysis, and reporting.
On-Call Rotation: Participate in off-hour/weekend call rotations (if applicable).
Professional Development: Continuously grow your professional knowledge and skills in line with current case management standards and your licensure requirements.
Additional Responsibilities: Handle other assigned tasks and adapt to changes in program requirements.
What You'll Need to Have:

Current Illinois LCSW, LCPC, or LPC licensure.
Minimum of five years of experience in various healthcare settings or a Managed Care Organization.
Solid understanding of case management principles, healthcare management, and reimbursement components, with experience in motivational interviewing.
Excellent clinical judgment and strong verbal and written communication skills.
Strong organizational, problem-solving, and time management skills.
Ability to ensure timely completion of projects and assignments.
Ability to prioritize and adapt to rapidly changing business needs.
Ability to work independently and remotely with excellent multitasking skills.
Proficiency in software applications, including MS Word, Excel, Access, PDF, and Outlook.
Experience navigating multiple Electronic Medical Records (EMRs).
Reliable high-speed/secured home internet connection, a private home office, and backup internet options.

Competitive Compensation: $70,000.00 per year, with flexible daytime scheduling options (four 10-hour shifts, Monday-Friday, 6 am-6 pm CT, available after orientation). Final pay is based on experience and qualifications.
