About this Role:
The Quality and Population Health Management (QPHM) Program leads clinical quality, health equity, and population health work at DHCS, and was recently selected to participate in the Centers for Medicare & Medicaid Services (CMS) Transforming Maternal Health (TMaH) Model. This 10-year model aims to improve outcomes and experiences for mothers and their newborns by addressing physical, mental health, and social needs experienced during pregnancy, while also reducing disparities and overall program expenditures. DHCS will implement the TMaH Model in the 5 Central Valley counties of Kern, Fresno, Madera, Kings, and Tulare.
Under the direction of the TMaH Project Director, the Associate Governmental Program Analyst (AGPA) will serve as the TMaH Project Coordinator. This position plays a key role in supporting the program’s objectives by ensuring compliance with federal requirements, overseeing contracts, writing and submitting necessary reports and documentation to CMS, and working collaboratively with internal and external teams to ensure the successful implementation of the TMaH project.

This position may be eligible for telework up to three days per week and is required to report in-person and site-based a minimum of two days per week. The amount of telework is at the discretion of the Department and is subject to change consistent with DHCS’ Telework Program and Government Code sections 14200 – 14203. Telework is available to California residents, and proof of residency may be required.
Business travel may be required, and reimbursement considers an employee’s designated office and residence subject to regulations and bargaining unit contract provisions.
