Job Overview:We are seeking a dedicated and compassionate Licensed Clinical Case Manager to join our remote team. In this role, you will play a key part in supporting members through Population Health Management programs by ensuring equitable, member-centered care. What You'll Be Doing:

Analyze data to determine member eligibility for Population Health Management programs.

Coordinate and deliver timely, effective, and member-centric care within HMO guidelines.

Manage case assignments, including outreach, documentation, progress monitoring, and case closure.

Ensure compliance with reporting and documentation standards while collaborating with team members and clients.

Assist members in achieving health autonomy by addressing barriers, social determinants, motivators, and psychosocial concerns.

Educate members on navigating the healthcare system and utilizing cost-effective, high-quality interventions.

Support operational goals and contribute to the success of the Population Health Management division.

Maintain confidentiality of sensitive information, including medical records and data.

Participate in Quality Management/Utilization Management Committee meetings as needed, including preparation, data analysis, and follow-up tasks.

Participate in an on-call rotation for off-hour/weekend calls (if applicable).

Engage in ongoing professional development and maintain a thorough understanding of case management standards.

Perform other duties as assigned based on program needs.

What You'll Need:

Licensure: Current IL LCSW, LCPC, or LPC.

Experience: Minimum of five years in healthcare settings or managed care organizations.

Knowledge: Strong understanding of case management principles, healthcare management, and reimbursement processes. Experience with motivational interviewing is preferred.

Skills: Excellent clinical judgment, verbal and written communication, and problem-solving abilities.

Organization: Ability to prioritize tasks, manage time effectively, and adapt to changing business needs.

Independence: Must be able to work remotely and independently while maintaining efficiency in a fast-paced environment.

Technical Proficiency: Experience using Microsoft Office (Word, Excel, Access, Outlook, PDF) and navigating multiple EMRs.

Home Office Setup: A secure, high-speed internet connection and a private home office with a locking door for confidentiality. Backup internet options may be required.

Compensation & Schedule:

Salary: $70,000 per year, paid bi-weekly.

Schedule: Flexible daytime hours (Monday-Friday, 6 AM-6 PM CT). Options for four 10-hour shifts available after a successful orientation period.
